Terms and Conditions of QB ACE LLC

1. Assumption of Risk:  I understand that participation in quarterback training involves inherent risks, including but not limited to the risk of injury, accidents, and unforeseen circumstances. I voluntarily assume all such risks.
2. Release & Waiver:  I release and discharge QB ACE LLC, Coach Caleb Arreola, and representatives from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation in the training session.
3. Payment Terms / No Refund Policy: All payments for quarterback training sessions provided by QB ACE LLC are non-refundable. By engaging in our training services, you acknowledge and agree to the following terms:

• Non-refundable Services: All fees and payments made for quarterback training sessions are non-refundable, regardless of the circumstances.
• Right to Terminate Services: QB ACE LLC reserves the right to terminate services for clients who do not comply with the terms and conditions outlined in the training agreement.
• Exceptions at Sole Discretion: QB ACE LLC reserves the right, at its sole discretion, to consider exceptions to this policy in extraordinary circumstances, such as medical emergencies, severe injuries, or other situations deemed exceptional by QB ACE LLC.

Acknowledgment: By engaging in our quarterback training services, you acknowledge that you have read, understood, and agreed to the "No Refund Policy" outlined by QB ACE LLC.

      4. Player Conduct Policy: Quarterbacks are required to bring cleats, running shoes, water, football, and the appropriate clothing for training. Quarterbacks should arrive ten minutes early to warm-up on their own.

      5. Training Behavior Policy:  Participants are expected to maintain professional and respectful conduct at all times during training sessions. Any form of disrespectful behavior, harassment, discrimination, or misconduct will not be tolerated.

          Trainer's Right to End Training: The trainer reserves the right to end the training relationship with a participant at their discretion if there is a violation of behavioral expectations or any form of misconduct.

• Examples of Misconduct: Misconduct includes but is not limited to: Harassment or bullying of other participants or the trainer.
• Use of offensive language or gestures.
• Failure to follow training guidelines.
• Any behavior that jeopardizes the safety or well-being of others.

     6. Photography and Videography Policy: I grant QB ACE LLC the right to take photographs and videos of me during training sessions for promotional and marketing purposes. I waive any right to inspect or approve the finished product.

    7. Medical Attention: I understand that QB ACE LLC does not provide medical insurance coverage for participants. In the event of any injury or medical emergency, I authorize QB ACE LLC to seek and secure medical attention on my behalf.

I have read and voluntarily signed this Athletic Training Release Form. I understand its content, and I am aware that I am waiving certain legal rights by signing this document.
